A&T Certified Home Care is dedicated to improving the physical, physiological, social, and spiritual development of people in the home care setting within the community and society. A&T is dedicated to provide, teach and support measures of comfort with dignity and expertise. The goal of A&T is to assist patients in achieving their optimal level of function in collaboration with their physician, the families, and significant others while maintaining a fiscally sound operation.
The Heart of Home Care
A&T Certified home care is a subsidiary of A&T Healthcare, which is family owned and operated by Registered Nurses since 1984. A&T Certified Home Care opened in 2011 and is Certified by the New York State Department of Health to service the Medicare and Medicaid population of Rockland County, NY.
A&T Certified Home Care is available 24 hours 7 days a week to provide patients with the opportunity to stay in their own home by providing, skilled care services including nursing, physical therapy, occupational therapy, speech therapy and home health aides as needed and authorized by their physician.
Our goal is to improve the psychological, social and spiritual development of people in their own homes within the community. A&T Certified also strives to assist patients in achieving their optimal level of function in collaboration with the patients, families significant others and the healthcare team.

Distinguished Health Care
A&T Certified Home Care provides RN’s, LPN’s, physical, occupational and speech therapists, medical social work, and home health aides.
Registered nurses work with and communicate with your hospital discharge planner and physicians to make the transition to home a smooth and professional one. If you call us directly from home to help yourself or a loved one, we will help you to determine the type of care you will need. We take great pride in working closely with you every step of the way.
